Infertility: Types, Causes, Symptoms, Diagnosis & Its Homeopathic Treatment




Introduction

To reproduce had been one of the main desired attributes of mankind for a long. It also defines their social status. Most couples start dreaming about their children immediately after marriage or within 2-3 years. 

But some couples fail in starting a family when they wish for it and in developing countries like India, couples have to face many difficulties related to reproduction. Especially, the position of women is defined by their reproductive capacity and their failure to do so due to any reason leads to social and cultural repercussions for them. 

Women express consistently lower scores on mental health, social functioning, and emotional behavior domains in quality of life and experience higher levels of stigma than men. They are called “Banjh” by others and there is no respect for her.

Infertility

Infertility is a disease or a condition when a couple fails to conceive even after 12 months or more of regular unprotected sexual intercourse without the use of any procedure of birth control.

It may be that any one or both the partners cannot contribute to conception, or that a woman is unable to carry a pregnancy to full term.

Prevalence 

About 10% to 14% of the Indian population is suffering from infertility, as per a report, given by ‘Indian Society of Assisted Reproduction’(ISAR). 

This rate of infertility is high among the urban population, with one out of six couples experiencing the problem, according to the report. Worldwide, 8 to 12 percent of couples experience the problem of infertility (WHO, 1991).

Types of Infertility

There are 2 types of infertility


Primary infertility:  This refers to couples who never become pregnant even after 1 or more years of having unprotected sex without using birth control methods.


Secondary infertility: This refers to those couples who previously have been able to get pregnant at least once, but now are unable.

Causes

There are many physical and emotional factors responsible for infertility. It may be due to problems in the man, woman, or both.



Infertility due to the problem in females 

  • An-ovulation – It is a condition when the ovaries do not release an ovum during a menstrual cycle. This is one of the biggest reasons for female infertility.
  • Blocked Fallopian tubes – Fallopian tubes are a pair of hollow structures which connects the uterus to the ovaries and where fertilization occurs.

       Blockages or scarring of these tubes leads to a lack of fertilization of sperm with the ovum.
  • Endometriosis It is a chronic and painful condition where the lining of uterine tissue is found outside the uterus. It may be found in the Fallopian tubes, cavities of the ovary, bowel, urinary bladder, and inside the peritoneal cavity on the ovaries. The scar tissue made due to this condition may cause infertility.

      It is characterized by the appearance of many small cysts in the ovary as seen on an ultrasound.
  • Fibroids – Fibroids are very common benign (non-cancerous) overgrowth that develops in the muscular wall of the uterus. Sometimes fibroids can be asymptomatic, but most of the time due to their size and location, fibroids can lead to problems for some women like abdominal pain and heavy bleeding. Fibroids also can cause infertility and recurrent miscarriages.
  • Premature Ovarian Failure - Women who are 40 years old or younger and whose ovaries no longer function to ovulate eggs are diagnosed with premature ovarian failure (POF). These young women potentially lose their reproductive capabilities.
  • Older ladies, who want to conceive – It is difficult for women over 40 to get pregnant, even if they are ovulating and having a period.

Secondary Infertility 

Even having one or more children, many couples faced infertility for various reasons.

Some rare causes of female infertility

  • Autoimmune disorders, such as antiphospholipid syndrome (APS)
  • Birth defects that cause defective reproductive tract
  • Cancer or tumor of the uterus or ovaries
  • Systemic diseases like uncontrolled Diabetes and Thyroid disorders
  • Consuming too much alcohol
  • Excessive smoking
  • Too much Exercise
  • Malnutrition or extreme weakness
  • Some medicines such as chemotherapy drugs
  • Hormone imbalances
  • Obesity
  • Old age
  • Pelvic inflammatory disease (PID)
  • Sexually transmitted diseases (STDs)
A woman is most fertile in her early 20s. The chances of a woman getting pregnant greatly decrease after the age of 35 and more. The age when fertility starts to decline varies from woman to woman. Infertility problems and miscarriage rates increase significantly after 35 years of age.

Infertility due to the problem in MALE



Some common causes of Male infertility are:

  • Impotency / lack of erection
  • Low Sperm count
  • Blockage in the duct which carries sperm
  • Poor or defective quality of sperm
  • Heavy alcohol consumption
  • Excessive Smoking
  • Drugs abuse
  • Hormonal imbalance
  • Systemic diseases like Diabetes
  • Sexually transmitted diseases
  • Prostate problem
  • Cancer treatments, including chemotherapy and radiation
  • Continuous exposure to high heat for prolonged periods
  • Obesity
  • Older age
  • Injury, or any surgery of the testes or nearby region

Risk Factors Responsible for Infertility

There are some lifestyle and health factors that can increase infertility:
  • Older age (being older than 35 (for women)
  • Overweight or Underweight
  • Excessive exposure to environmental toxins, like lead or pesticides
  • Excessive alcohol intake or drug abuse
  • Smoking or tobacco chewing
  • A history of pelvic inflammatory disease (PID)
  • Injury to the scrotum and testes
  • Tight undergarments may lead to the overheating of testicles
  • Having an undescended testicle

Diagnosis

Diagnosis mainly depends on medical history, any medication, sexual habits, and physical examinations of both partners. To confirm the diagnosis some laboratory tests and the radiological test may be needed.

Infertility tests for men

  • Semen analysis: To know the quantity and quality of sperms.
  • Blood test: To know the levels of testosterone and other hormones.
  • Ultrasound: To know about any obstruction in the ejaculatory duct or retrograde ejaculation.
  • Chlamydia test: To rule out Chlamydia infection.

    Infertility tests for women

  • Blood test - To assess the hormonal imbalance in a woman, if any.
  • Laparoscopy – This test has to be done to investigate any signs of endometriosis, blockages, scarring and other problems in the uterus and Fallopian tubes.
  • Pelvic ultrasound - To rule out any abnormality in the pelvic organs.
  • Hysterosalpingography - Fluid is injected into the woman's uterus and X-rays are taken to determine any blockage in the Fallopian tubes. If a blockage is present, surgery may be needed.
  • Genetic testing - To see any genetic abnormality which may be interfering with fertility.
  • Chlamydia test - To rule out Chlamydia infection.
  • Thyroid function test - To rule out any hormonal imbalance.

Treatment

Treatment depends on the cause of infertility. To treat infertility, first, we have to treat the underlying cause. Treatment also depends on the general state of health of the couple. 

The couple may be advised to have sexual intercourse more often around the time of ovulation to increase the chances of fertilization of sperm with egg.

Complications

Infertility treatment can cause some complications. Unsuccessful attempts for a long time can lead to stress and depression in couples.

Ectopic pregnancy

This is when a fertilized egg implants outside the womb, usually in a Fallopian tube. If it stays in there, complications can develop, such as the rupture of the Fallopian tube. This pregnancy has no chance of continuing. It is a surgical condition.

Women receiving fertility treatment have a slightly higher risk of Ectopic Pregnancy. An ultrasound scan can detect an Ectopic Pregnancy.

Repeated blood tests and ultrasounds have not revealed any pathological problems. Her gynecologist was not having any idea why this has happened.

Unpleasant skin growths like Warts & Corn are a very common problem but are highly troublesome & painful sometimes.

Scientists estimate that about one in ten people has warts or verrucas at some point in time in their lives. Warts and corns are the most common skin problem after acne.

What are warts?

Warts are annoying skin growths that are caused by the excessive and rapid growth of keratin, which is a hard protein on the top layer of the skin. Sometimes they go away within months or years.

Causes of Warts

Warts are caused by a virus called ‘Human Papilloma Virus’ (HPV). There are more than 60 kinds of HPV, out of these; some are responsible for warts on the skin. 

HPV virus infects the superficial layer of the skin and enters the body from the broken surface of the skin. HPV stimulates the quick growth of cells on the skin's outer layer, forming a wart.

Risk factors for Warts

Although we are all exposed to HPV, some people seem to be more susceptible to it, while others are naturally resistant. Warts can appear more often in:
  • People with weakened immune systems.
  • Children and teenagers, as have not (yet) built up antibodies to the virus and have more frequent contact with other wart carriers.
  • People who are on steroid treatment, or who are suffering from the disease like HIV infections, which suppress the immune system of the body.
  • People who walk barefoot where exposure to a wart-causing virus is common, such as in locker rooms.
Genital warts are more contagious. Different HPV strains cause different warts.

Risk of Spread of Warts

They may occur in any age, sex & society. They can happen anywhere on the body, are disfiguring and embarrassing, are highly contagious, and can also spread from one part of a body to another. 

They may be painful if they are located in areas of repeated friction or pressure. The virus can spread to other parts of the body through:
  • Scratching or biting a wart
  • Sucking fingers
  • Cutting or biting the nails, if there are warts around the nails
  • Shaving the face or legs
Having wet or damaged skin, and coming into contact with rough surfaces increase the risk of infection.

Types of warts

There are different types of warts depending on location and their structure:

Common wart (Verruca vulgaris) - 

These warts have a firm, raised, rough surface and may appear cauliflower-like.


They can occur anywhere on the body but mostly appears on hands, knuckles, fingers, elbows, knees, and any area with broken skin. 

Flat wart (Verruca plana) - 

Flat warts are small, smooth flattened warts, flesh-colored, which can occur in large numbers; most common on the face, neck, hands, wrists, and knees.


Also known as plane warts, they grow most often in sun-exposed areas.

Filiform or digitate wart -

These are thread- or finger-like warts, long and thin in shape, most common on the face.


They can grow rapidly on the eyelids, neck, and armpits.

Genital wart (venereal wart, Condyloma acuminatum, Verruca acuminata) - 

A wart occurs on the genitalia. Most warts are harmless, but some strains of HPV can cause warts on, in, or around the genitals. These warts can be more serious. In women, they can potentially lead to cervical, anal, and vulvovaginal cancer.
In men, anal cancer and cancer of the glans penis can also occur as a result of infection with some types of genital warts.

  • Mosaic warts - These are multiple plantar warts in a large cluster, commonly on the hands or soles of the feet.

  • Periungual wart - A cauliflower-like cluster of warts that occurs around the nails.

  • Plantar wart (Verruca plantaris) - These are painful (in some cases) and hard lumps, usually only appearing on pressure points on the soles of the feet. Painful verrucae appear on the soles of the feet, heels, and toes.



Plantar warts are often difficult to clear.

Symptoms of Warts

Symptoms mainly depend on the type of warts. They are usually painless. But at pressure sites, they can be painful.

How to diagnose warts?

The physical appearance of a wart can suggest the diagnosis and type of wart. In a rare cases, it can be confirmed by skin biopsy by taking a sample of the wart. It is needed if it isn't clear that the growth is a wart or if it bleeds and is fast-growing.

It is a good idea to see a doctor if the wart:
  • Causes pain
  • Bleeds easily
  • Changes appearance
  • Spreads easily to other parts of the body
  • Comes back after prior removal
  • Needs to be removed for cosmetic reasons.

Treatment of Warts & Corns

Warts are usually harmless, but in case they are problematic and need treatment, the choice of treatment includes:
  • Using a home treatment such as salicylic acid or duct tape. You can get these without a prescription.
  • Putting a stronger medicine on the wart, or getting a shot of medicine in it.
  • Cryotherapy - Freezing the wart.
  • Removing the wart with surgery by electro-surgery, curettage, or laser surgery.
Wart treatment doesn't always work. Even after a wart shrinks or goes away, warts may come back or spread to other parts of the body. This is because even if you remove warts by surgery or salicylic acid, the virus remains in the body that causes the wart.

How to prevent warts & Corns?

The first way to prevent skin warts is to avoid coming into contact with the virus and reduce the risk of catching or spreading warts:
  • Do not touch other people's warts.
  • Do not use the infected person's towels, washcloths, or other personal items.
  • Do not share shoes and socks with the infected person.
  • Do not scratch or touch warts repeatedly, as this can cause them to spread.
  • Wear sandals when entering and exiting communal showers and pools.
  • Do not use the same nail cutter on the infected nail and the healthy nails.
  • Do not bite fingernails if warts are near them.
  • Keep hands as dry as possible.
  • Wash hands thoroughly after touching a wart.
  • Also, when if you go to the barber for shaving, be sure to use a new razor that won’t tear or cut your skin, and don’t use their towel, etc.
Warts are common, and they can cause embarrassment. Sometimes they need treatment to eliminate them.

Case 2

This is a case of a male of 25 years old, which was having a large horny plantar wart on the plantar surface of the left palm. It was too big and painful if something hit with this. The patient was unable to close the fist completely as it was painful.


After a complete assessment of his symptoms we prescribed the homeopathic medicine to him and in two months, that planter wart had gone completely.

Case 3

This is a case of a small girl 7 years old, which was having a big wart on her tongue. The girl feels very uncomfortable due to the presence of this wart on her tongue.

What is Jaundice?

The word jaundice is derived from the French word “jaundice”, which means "yellow disease". 

Jaundice, also known as icterus ('Kamla rog' or 'Peeliya' in Hindi), is a term used to describe a yellowish pigmentation of the skin and whites of the eyes (sclera) which is caused by an excess of bilirubin in the blood (hyperbilirubinemia). The feces may be pale and the urine dark.

The color of the skin and sclera varies depending on levels of bilirubin; mildly elevated levels display yellow skin and sclera, while highly elevated levels display brown.

Jaundice itself isn't a sickness, but instead an indication of one of the numerous underlying pathological processes that happen eventually along the ordinary physiological pathway of the metabolism of bilirubin in the blood.

What is Bilirubin:-

It is a yellow-colored substance and a waste product that formed due to the breakdown of RBC when the iron is removed from hemoglobin in red blood cells. Excess bilirubin leaks out into surrounding tissues causing yellowness of the skin and tissues. 

The Two forms of bilirubin can be measured by laboratory tests:

• Unconjugated bilirubin - when heme is discharged from hemoglobin, it is changed over to unconjugated bilirubin. It is conveyed by proteins to the liver. Little sums might be available in the blood.

• Conjugated bilirubin - framed in the liver when sugars are connected (conjugated) to bilirubin. It enters the bile and goes from the liver to the small intestine and is in the end disposed of in the stool. Ordinarily, no conjugated bilirubin is available in the blood.

Normal Levels of Bilirubin:

Total 0.2 – 1.2 mg/dL in adults (It is a combination of direct and indirect bilirubin.)
Direct bilirubin is 0.1-0.4 mg/dL.

Types of jaundice

There are three main types of jaundice:

Pre-Hepatic Jaundice (Hemolytic Jaundice)when there is an excessive breakdown of red blood cells (hemolysis) occurs, it leads to an increase in the production of bilirubin. It can be caused by some conditions like sickle cell anemia and hemolytic anemia.
Intra-hepatic jaundice (hepatocellular jaundice) – occurs as a result of liver disease or injury. It can be caused by some diseases such as Gilbert's syndrome, cirrhosis of the liver, or other liver damage.
Post-hepatic jaundice (obstructive jaundice) – occurs as a result of an obstruction in the bile duct (a system of tubes that carries bile from the liver to the gallbladder and small intestine), which prevents bilirubin from leaving the liver. it's caused by conditions such as gallstones or tumors.

Causes of jaundice

Jaundice regularly happens because of a fundamental issue that either causes overproduction of bilirubin or prevents the liver from disposing of it, both of which result in bilirubin being kept in tissues.
  • Acute inflammation of the liver
  • Inflammation of the bile duct - may prevent the discharge of bile and expulsion of bilirubin, causing jaundice.
  • Blockage of the bile duct – This obstruction prevents the liver from disposing of bilirubin.
  • Hemolytic anemia - production of bilirubin increases when large quantities of red blood cells are broken down.
  • Gilbert's syndrome - an acquired condition that disables the ability of enzymes to process the excretion of bile.
  • Cholestasis - a condition where the stream of bile from the liver has interfered. The bile containing conjugated bilirubin stays in the liver as opposed to being discharged.

Some rare conditions also can cause jaundice are:

  • Crigler-Najjar syndrome - an inherited condition that impairs the specific enzyme responsible for processing bilirubin
  • Dubin-Johnson syndrome - an acquired type of chronic jaundice that prevents conjugated bilirubin from being discharged out of the liver's cells.
  • Pseudojaundice - a relatively harmless type of jaundice in which the yellowing of the skin occurs due to an excess amount of beta-carotene, not from an excess amount of bilirubin. This condition results from eating lots of carrots, pumpkins, or melons.

Symptoms of jaundice



•    Yellowness of the skin and the whites of the eyes can be examined
•    Pruritis (itchiness)
•    Fatigue
•    Abdominal pain – Due to the obstruction of the bile duct
•    Weight loss
•    Vomiting
•    Fever
•    Paler than usual stools
•    Dark urine

Diagnosis of jaundice

Normally Physicians diagnose jaundice by asking about the patient's history and by examining the patient physically, paying close attention to the abdomen. Doctors will check the hardness of the liver; a firm liver suggests cirrhosis, while a stony hard liver indicates cancer.

The severity of jaundice is determined by several tests, such as liver function tests and other blood tests. Some of these tests include:

  • Bilirubin tests - a high level of unconjugated bilirubin relative to levels of conjugated bilirubin indicates hemolysis (accelerated breakdown of red blood cells)
  • Complete blood count (CBC) - measures levels of red blood cells (RBC), leukocytes (WBC), and thrombocytes (platelets)
  • Hepatitis A, B, and C tests
  • If an obstruction of the liver is suspected, the liver's structure will be looked at with the help of imaging tests. Some of these tests include:
  • MRI scan - uses magnetic signals to create image "slices" of the soft tissues of the human body.
  • Abdominal ultrasonography (ultrasound) - uses high-frequency sound waves to create a two-dimensional image of the soft tissues of the human body
  • CT or CAT scan - uses a thin X-ray beam to create image "slices" of soft tissues in the body
  • Endoscopic retrograde cholangiopancreatography (ERCP) - a procedure that combines an endoscopy and X-ray imaging
  • A liver biopsy may be required for checking the inflammation, Liver cirrhosis, cancer, and fatty liver.

Treatments for jaundice

Treatment of jaundice depends on the actual cause which results to jaundice.

  • Jaundice caused by Anemia may be treated by providing iron in the blood externally; either by taking iron supplements or eating more iron-rich nourishment.
  • Hepatitis-induced jaundice may be treated with anti-viral or steroid medications.
  • Obstruction-induced jaundice may be treated via surgery to remove the obstruction.
  • Drug-induced jaundice is dealt with by choosing an option prescription and by ceasing medicines that caused jaundice.

Complications from jaundice

There may be severe itching that occurs which can lead to scratching of the skin raw. Most of the complications that arise are due to the underlying cause of jaundice, not from jaundice itself.

Prevention of jaundice

Jaundice is related to the function of the liver, so it is necessary that people keep up this crucial organ's well-being by eating an adjusted eating regimen, practicing consistently, and ceasing from surpassing suggested measures of liquor.

Neonatal jaundice




Usually, Neonatal jaundice is harmless: this condition arises in infants around the second day after birth, lasting until day 8 in normal births, or to around day 14 in premature births. 

Serum bilirubin generally decreases to a low level without any intervention required. If bilirubin levels in babies are very high for too long, a type of brain damage, known as kernicterus, may occur, leading to significant disability

Typical causes for neonatal jaundice are Physiological jaundice, jaundice due to ready-made food supplements, and hemolytic blood disorders including, hereditary spherocytosis, glucose-6-phosphate dehydrogenase deficiency, pyruvate kinase deficiency, ABO/Rh blood type auto-antibodies, or infantile pyknocytosis.

A Bili light or 'Jaundice light for newborn baby', is used for early treatment, in which the baby is exposed to intensive photo-therapy. Sunbathing is also an effective treatment and has the advantage of ultra-violet-B, which promotes Vitamin D production. Bilirubin count is lowered through bowel movements and urination, so frequent and effective feedings are especially important.
